Practice Period 2025
To join the Practice Period, please complete and submit the Practice Period Enrollment Questionnaire, below.
INSTRUCTIONS
First, read the main Practice Period 2025 webpage, paying particular attention to the schedule and commitments.
Then, review the Enrollment form below and compose your responses. We suggest that you draft your answers to the long-form questions in another document, then copy and paste them into the form when you are ready. The form will not save your progress and changes could be lost if the connection is interrupted.
Finally, complete and submit the questionnaire.
After submitting the questionnaire, you will receive a copy of your completed questionnaire via email. If you do not receive it within a few minutes, please check your spam filter. If you are a Gmail user, please also check under the Updates and Promotions tabs and under All Mail.
The suggested donation for Practice Period is $150. If you have not yet made a donation, please donate now.
